Job Description
The RN- Case Manager plans, organizes and directs hospice care with emphasis on community health education/experience. The position also builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
• Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Traveling throughout the day and after hours is expected
• This position will also work an on-call schedule on a rotating basis

Requirements:
1. Must have current Registered Nurse (RN) license in state(s) of practice.
2. Graduate of an accredited school of nursing required. Associate degree required. Bachelor’s degree preferred.
3. One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred.
4. Ability to pass DHS background study.
5. If Driving: Valid State Driver’s License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
