Job Description:

Under the supervision of the Hospice House Inpatient Supervisor, the Licensed Practical Nurse is responsible for providing nursing and supportive care for the terminally ill patient and their families in accordance with agency policies. We ask you to support us in achieving our Mission and Vision by contributing the following: Responsible for implementing the care plan for patient/family unit. Communicates with members of the Interdisciplinary team (IDT) as needed regarding patient status and to coordinate and ensure consistent high quality care. Provides direct nursing care to patients in order to maintain highest level of care for the patient under direction of the primary physician and registered nurse. Assesses continuing educational needs and provides ongoing education and emotional support to patients, families and caregivers. Attends and participates in daily report, team meetings, briefings, in-services, department and staff meetings, and other related activities. Communicating effectively and professionally within the team environment, demonstrating respectful and appropriate language and nonverbal behavior; actively seeking to resolve customer and co-worker concerns in a timely manner. Demonstrating the ability to recognize, understand, and appreciate our commitment to Diversity, Equity and Inclusion (DEI), by demonstrating social and cultural sensitivity appropriate to diverse patients, families, and staff.

Requirements:

The Ideal Candidate Will Possess the Following Requirements: Licensure as a Practical Nurse in Missouri. Acute care or Long Term care experience preferred. Home Health or Hospice experience preferred. Licensed driver with reliable transportation. CPR certification required. May obtain after hire.
