Transportable Physician Orders for Patient Preferences Live Interactive Teleconference Outcomes:
At the end of this teleconference, participants will be able to:
- Identify the functional trajectories that patients with serious illness display in the months to few years prior to death.
- Apply serious illness care planning into provider workflows, for more, better, earlier conversations about goals of care and to recognize the benefit of using a serious illness care planning tool to guide such conversations.
- Determine the importance of making documentation of serious illness care planning conversations and documents accessible and retrievable in the EMR.
- Describe TPOPP (Transportable Physician Orders for Patient Preferences).
- Identify the Bistate infrastructure and resources to support development of TPOPP community coalitions in Missouri and Kansas.

This is a free live interactive teleconference but there will be a $10 fee for CE’s. Please complete the sign in/sign out sheet, evaluation, post test and return to MHPCA at 573-635-0659 or crystal@missourihospice.org. The certificates will be generated within 5 business days and emailed to the contact listed on the sign in/sign out sheet.
Please register before December 5th, 2016.

This program provides 1.5 MO Nursing Contact Hours. This program provides 1.5 Social Worker Hours. All Social Workers must complete a post test and return to MHPCA via fax (573-635-0659) or email to crystal@missourihospice.org to receive SW CE hours. Participants from Missouri interested in credit hours should indicate on the sign in sheet from each hospice their desire to receive credit hours. There will be an additional $10.00 per person for contact hours. Please make checks payable to MHPCA (P.O. Box 105318, Jefferson City, MO 65110) with a copy of your sign in sheet to receive credit for participants. All participants wanting CE credits must sign the sign in/sign out sheet, fill out an evaluation, complete a post test (Social Worker’s only) and return to MHPCA at the end of the Live Interactive Teleconference via fax (573-635-0659) or email to crystal@missourihospice.org. The certificate will be generated and sent to the contact on the sign in/out sheet via email with in 5 business days.
