Judy Morris, RN, BSN, HFNC, 5 Star Consultants
Over 45 years as registered nurse, with the last 20 years including home health, coordination of SNF with hospice, QAPI, worked 15 years for Missouri Bureau of Home Care as hospice/ home health surveyor over 12 years, then promoted to Assistant Bureau Administrator — supervised a team of hospice/ home health surveyors for 3 years. Provided consultation with hospice agencies on regulatory issues.  Joined 5 Star Consultants in January 2019. Performed consultations onsite at hospice agencies, conducted mock surveys, provided education of new staff/ administrators, and performed QAPI audits since 2019.

Overview:
Is your hospice compliant with the new requirements for revised election statements with timely provision of election addendums? What about consents for patient rights, given at admission?  Does your hospice document level of care changes and appropriate use of each type of level of care (such as continuous care or GIP) to avoid payment denials?  Does your documentation show coordination, decisions and IDG discussions about discharges (deaths, extended prognosis, or discharge for cause)?  Does your IDG care planning/ coordination documentation prevent serious condition level deficiencies? Join us for this informative webinar as we discuss each of these areas in detail so you can ensure your understanding of Hospice A (admit) to DC (discharge).

Objectives:
At the end of this educational initiative, participants should be able to:

  • Outline how to ensure consent forms are complete and show compliance with patient rights regulatory requirements
  • Examine how to ensure complete/ current and individualized plans of care, documentation to show the plan of care is delivered as written or updated as needed; and sufficient documentation of coordination by IDG
  • Define each type of hospice level of care
  • Describe appropriate documentation/implementation when level of care changes occur
  • Explain the differences between revocation and discharge
  • Describe appropriate documentation for revocation, transfers and discharge – in particular for discharge for cause

This webinar is $110 for members and $130 for non-members.

This program provides 1.5 MO Nursing Contact Hours. This program provides 1.5 Social Worker Hours. All Social Workers must complete a post test and return to MHPCA via fax (573-635-0659) or email to crystal@missourihospice.org to receive SW CE hours. Participants from Missouri interested in credit hours should indicate on the sign in sheet from each hospice their desire to receive credit hours.  There will be an additional $10.00 per person for contact hours.  Please make checks payable to MHPCA (P.O. Box 105318, Jefferson City, MO 65110) with a copy of your sign in sheet to receive credit for participants.  All participants wanting CE credits must sign the sign in/sign out sheet, fill out an evaluation, complete a post test (Social Worker’s only) and return to MHPCA at the end of the Live Interactive Teleconference via fax (573-635-0659) or email to crystal@missourihospice.org. The certificate will be generated and sent to the contact on the sign in/out sheet via email with in 5 business days.
