Objectives:

This session is intended to discuss the beginning to end coordination of Hospice and Nursing Facilities working together.

  • Bringing Hospice into the facility – How to know when it’s time for hospice?
  • How does hospice benefit the facility, resident & family?
  • Finance & Medications – how does billing work? What medications will need to be cut?
  • Care Planning – How hospice and the facility can work together to develop a care plan for the resident?
  • End of Life Decisions.
  • Grief Counseling for family, and facility employees.

The cost for this seminar is $150.00 for members & $200.00 for non-members.

Missouri Hospice & Palliative Care Association, #1359, is approved as a provider for social work continuing education by the Association of Social Work Boards (ASWB) www.aswb.org, through the Approved Continuing Education (ACE) program. Missouri Hospice & Palliative Care Association maintains responsibility for the program.  ASWB Approval Period: 2/28/16 – 2/28/17.
