Objectives:
This session is intended to discuss the beginning to end coordination of Hospice and Nursing Facilities working together.
- Bringing Hospice into the facility – How to know when it’s time for hospice?
- How does hospice benefit the facility, resident & family?
- Finance & Medications – how does billing work? What medications will need to be cut?
- Care Planning – How hospice and the facility can work together to develop a care plan for the resident?
- End of Life Decisions.
- Grief Counseling for family, and facility employees.
The cost for this seminar is $150.00 for members & $200.00 for non-members.
