Speakers:

Tammy Forebeck, BSW, MEd -Tammy earned her BSW from Illinois State University & med in general Counseling from UMSL. She was a nursing Home Social Worker for 2 years, then moved into hospice in 1994. Tammy started working for Beacon of Hope Hospice first and then did PRN Social Work for various hospices. She then started with Mercy Hospice in 1998 as PRN and then Part Time in 1999.Tammy is the chair for GSLHO SW Group and the chair for MHPCA SW Group.

Christi Lero, MSW – Christi graduated with a Master of Social Work from the University of Missouri in Columbia, Mo. During her graduate career she participated in various research projects, one of which was presented at the Social Work Hospice & Palliative Care Network Assembly in Scottsdale, AZ. Psychosocial Issues Affecting Seriously Ill Patients of South Asian Origin is under review for publication by the Palliative and Supportive Care Journal. As a research assistant in Family and Community Medicine, Christi assisted with the following projects:

• National Cancer Institute Problem Solving Intervention for Family Caregivers in Palliative Oncology Pilot Study
• Patient-centered end of life care for culturally diverse populations: Needs, wants, and care for South Asian populations with Dr. Nhidi Kosla and Dr. Karla Washington
• Palliative Care Quality Improvement Measures at Ellis Fischel Cancer Center
• Wrote and recorded Guided Imagery tools using mindfulness practices, and developed educational pamphlets for tool introduction

Since graduating in 2016, Christi has worked as a Hospice Social Worker at Hospice Compassus & Palliative Care, where she continues to hone clinical skills and is currently under clinical supervision to obtain a License Clinical Social Work (LCSW). Christi has maintained network and academic connections and has participated in guest panel lectures and individual lectures on the state of end of life care, hospice, and palliative care. She served as a presenter in the 2017 Caring for the Frail Elderly Conference, and in 2018 at the University of Central Missouri 29th Annual Social Work Conference where she presented non-pharmacological end of life comfort measures. Christi maintains membership with the National Association of Social Workers (NASW) where she is a Political Action for Candidate Election (PACE) committee member.

Christa Burke, MSW, LCSW, ACHP-SW – Christa has a Master’s degree in Social Welfare from UCLA and is a Licensed Clinical Social Worker. She obtained Advanced Certification in Hospice and Palliative Care Social Work (ACHP-SW) in 2015. She has a broad background in social work but began medical social work as a Pediatric Oncology Social Worker at Dartmouth Hitchcock Medical Center in 2005.  She transitioned to adult palliative care in 2015.  Christa is currently a Palliative Care Social Worker with the Washington University Division of Palliative Medicine at Barnes-Jewish Hospital in St Louis, MO.  Christa serves as the Social Work Consultant for the Missouri Hospice and Palliative Care Association (MHCPA) and is a member of NASW, SWHPN (Social Work Hospice and Palliative Care Network) and the St Louis Social Work Leaders in Healthcare organization. Through MHPCA, Christa serves as a Hospice educator for in Missouri Correctional Facilities teaching offenders to serve as hospice volunteers to other offenders in need.  Christa is passionate about helping patients and their families cope with serious illness and encouraging meaningful discussions about the experiences and values that give meaning to a person’s life.  She currently works on a dynamic interdisciplinary team in both an inpatient and outpatient setting focusing on the needs of the whole person. She enjoys teaching social work students, medical students, residents and fellows in addition to her direct patient care.  She has recently published an article about her Palliative Care MSW Practicum Curriculum designed to educate future social work leaders in Palliative Care.

Joshua Mazur, MSW, LCSW, ACHP-SW – Josh Mazur began his career in end-of-life care and palliative care as a volunteer with BJC Hospice in 2012. In 2105 he earned his MSW from the School of Social Work at Saint Louis University with a concentration in health and mental health and a specialization in gerontology. Josh did his practicum with the St. Louis Area Agency on Aging, BJC Hospice in its adult hospice program, and with Barnes-Jewish Hospital’s Inpatient Palliative Care Services Team. Shortly after graduation BJC Hospice hired him on as a hospice social worker and he’s remained for the past six years. In 2019 Josh earned his LCSW and completed the requirements for the advanced certified hospice and palliative social worker (ACHP-SW) through NSAW & NHPCO. Josh has been preceptor for numerous social work new-hires at BJC Hospice and so far has had one practicum student. For BJC Hospice Josh has also served on several committees/working groups including the Patient and Family Advisory Council, the start-of-care improvement committee, and We Honor Veterans, among others. He’s also volunteered for BJC Hospice’s free bereavement camps and retreats for children and teens for ten consecutive years. Finally Josh currently assists with psychosocial assessment and follow-up with the Barnes-Jewish Hospital Virtual Outpatient Clinic for Palliative Care. Josh lives in Chesterfield, MO with his wife Abbi and their Bernese Mountain Dog Shayna.

Outcomes:

The overall objective for attendees is education in   hospice regulations, levels or care, as well as  nursing home requirements and regulations. Attendees will learn how to develop a thorough hospice psychosocial assessment, identify short term therapeutic interventions and how to       develop a social work plan of care in the hospice setting. Instructors will explain the team concept in the hospice setting and the role of the social worker, as well as describe a normal grief process. Speakers will explore documentation and identify areas of stress and develop a plan of care for our own wellbeing. This is a basic course for the      beginning social worker with less than two years experience.

*This training includes a take home reference manual for each attendee to use as a resource.*

This seminar provides 7.5 Social Worker Contact hours.
Social Worker CE’s are available for $25 each.

Upon completion of this course each attendee will be required to complete an evaluation. Each participant will then receive a general certificate unless a CEU is purchased. Each attendee must complete the entire course to receive CE credits. Certificates will be distributed to attendees upon signing out of the seminar and returning the sign in sheet and evaluation to MHPCA.
